Using HP diagnostic tools, I checked the hard drive...
Can anyone explain what this failure ID means and what to do about it?

Those codes do little more than provide the serial number of your PC.
Here is the failure report generated by the code you posted, minus your notebook's serial number.
Failure Information

It would appear to me that in order to fix the problem, you would have to remove and replace the SSD.
