My Envy laptop shuts off immediately when I pull the AC adapter out. When it is operating, the battery is not charging and staying at 0%
battery test Results from HP PC diagnostics app say Cancelled.
Battery Test Results from startup (esc, using F2 method) shows FAILED with a FAILURE ID:
RLP2KW-85VA7N-MFPX7F-C0FR03
Charge Status: 94%
Battery Status: Replace (40)
Warranty Type: 1
Battery Age: 1518 days (I bought laptop brand new 3 years ago)
Full charge capacity: 2686 mAh
Remaining capacity: 0 mAh
My questions are:
#1: Is there any calibration or diagnostic I can run to fix this? Or is a new battery necessary.
#2: What would even cause this type of failure? Like I said, I bought brand new exactly 3 years ago and I don’t use it often. I do keep it plugged in most of the time, though. Does that affect it?

You simply need a new battery. 3 years is a normal lifespan regardless of how strenuously it was used. If we could have the full model or product number of the laptop we can help you find a replacement. HP will almost surely no longer carry it and you are going to have to purchase in the aftermarket.
