You have to open you device manager:

1) Open Windows File Explorer.

2) Right click on "This PC".

3) Click on "Manage".

4) CLick "Device Manager".

 

There you'll see all your devices, you must see the bluetooth logo, you have to expand the list of BT devices clickin on the cross o arrow, so you will see the whole list of BT devices.

 

You have to take a screenshot of that, how to take a screenshot? You can use the tool "Snipping tool", it comes with Windows 10. To open it you can click on your windows logo from Task Bar and direcly type "snipp" then it'll show you the tool, click on it.

 

The tool is pretty easy to use, click on New botton and it will allow you to take a screenshot of the screen.

 

I hope that helps you to attach a screenshot.
