I found an older thread in this forum that mentioned these Event 258 errors for the IntcAzAudAddService, and wanted to post a solution for these SYSTEM Event Log errors and accompanying lack of audio or video playback.  Note: the old thread was closed so I'm opening this new one. 

 

A solution (for Windows 10/11):

 

  • Run Device Manager
  • Expand the Sound, video and game controllers item
  • Right-click Realtek Audio and choose Properties
  • Click the Driver tab
  • Click the Uninstall Device button.  At the warning screen DO NOT check the "Delete driver from this device" option.  Leave the option unchecked, then proceed with the Uninstall.
  • The Realtek Audio device should no longer appear in Device Manager under Sound, video and game controllers.
  • From Device Manager's menu system, go to Action > Scan for hardware changes.  Windows should detect the driverless Realtek Audio device and automatically reinstall the proper driver.

In my case I did not need a new driver, i just needed to uninstall the driver and then reinstall it per the procedure, above.
