The issue could be with the Hard Drive, try testing it by running a Hard Drive Self Test.

Power up the notebook, tap F10 key on startup, and at the BIOS Setup Screen, select Diagnostics >> Primary Hard Drive Self Test.

Let the Short and Long Test complete. If you receive any error with the test, that would confirm a Hard Drive failure.

Please provide the exact notebook Model and Product Number with the results.

after the test,it says

Test Finished:Failed

Test Status: 7(Completed with the read element of the test failed)

You will have to replace the hard drive. A retail 2.5" SATA hdd can be used.Be sure to get one of at least as large a capacity as the original to use your Recovery Disks. It can be larger but not smaller..for instance if you have a 320gb hdd it could be replaced with 500gb but not 250gb.

Hi Gbrenda,

I understand you are getting a disk read error occured and then have to press ctrl+alt+del to restart. A hard or forced reset may restore functionality. When performing a hard reset, you must disconnect or remove all peripheral devices.

Hope this helps.
