The most common cause is failure to set the paper size and type on the printer itself. If that does not match the actual loaded media and the paper size and type that the printer driver is formatting the print job for. For example, if you are trying to print a letter size document on plain paper, you need to load plain letter paper and then set the paper size and type on the printer to match that.
