Should I apply this and what will it deliver? Printer is 9 years old and works well. Have just successfully updated software to restore scanning under Catalina. Firmware never knowingly updated before, so am v wary of something going wrong.
Only issue now is scanning of highlighter pens - will this upgrade fix this?

If the printer works as expected, it is entirely up to you whether you update the Firmware.
HP Photosmart 6520 e-All-in-One Printer series Firmware Update
Notes say the update is for "network".
Were it my decision, I'd not do so unless the printer stopped performing as needed.

Will Firmware fix issues with printing highlighter pens?
No.
In fact, nothing in the software can affect this issue.
More likely, you just need to change the type of highlighter pen you are using.
The following is an older article from 2018 - the information is likely just as relevant today...
InkJet ink can smear when highlighted.
Consider using a “gel” type highlighter:
