I did Fax setup and ran test.  All Pass.  When I try to send fax the machine dials the number but then I here it say "A carrier access code is required"  Does the fax software supply or is it provided by my carrier?  Might it be a problem with the line that is connected to my residence.  I have DSL and have the machine connected with the adapter supplied by the provider as per fax instructions.  The fax is connected with the phone cable supplied by HP, 2 wire copper connector on each end.

I understand that you get a message "A carrier access code is required" when you try to send a fax from your HP OfficeJet 6962 All-in-One Printer.

 

This issue could occur if there is a code set by your carrier for long distance fax.
