Dear Support,
I have pre-printed CMS 1500/ HCFA forms but I can't get my printer to print them right. So I was wondering if there was anything I could change on my printer. Thank you for your help.

Hi @beachbia11,
I would be happy to help you print on the CMS 1500/HCFA forms. I suspect the printer paper sensor is having a hard time reading the paper and may be prompting you with some sort of a paper mismatch error? When you look at the print dialog box, what paper type to you have selected, and what options do you have to choose from?
I feel changing the paper type might help. Let me know what options you have, and we can proceed from there. Please also let me know what program you are printing from as well. Thanks.

Thanks for getting back to me with the requested information. Let's try something different to see what happens. Please print the PDF as an image and let me know if doing so works. Acrobat Help / Quick fix | Print PDF as image | Acrobat, Reader.

I tried to post a picture and a saved word doc but it states my file is to big. I did try to print as an image and it came out the same. I am not sure if you are familiar with the CMS 1500.
Box 1, 1a, 2, 3,4, 5 and 6 the prints below the box.
Box 10 prints to the right side
Box 20 - 30 prints to high in the box (some of it covers the preprinted information ex: box 25-33)

The more research I do, the more I think it might be beneficial for you to reach out to whom you got the form from to ask what techniques they suggest to print the form. Is the paper thinner than standard plain paper? Maybe that is why it won't feed? I originally thought the sensor was having difficulty detecting/sensing the paper properly due to the form data. I found this on a site when I Google searched the paper form:
The Centers for Medicare & Medicaid Services (CMS) approved a new CMS-1500 form (02/12) to replace the previous form (08/05). Fully updated with the latest revisions, the new CMS-1500 form is 100% compliant and printed to exact specifications for layout, paper and ink.
· Designed for laser printers or inkjet printers, can also be handwritten
· Uses soy-based black and OCR “dropout” red ink for greater scanning accuracy
· Printed on SFI (Sustainable Forestry Initiative) paper
· Features Anti-Rejection Projection™* -- guaranteed acceptance by insurance carriers for rejection-proof claim filing and quicker payment
Although it says it is designed for laser printers and inkjet printer, the template is printed on Sustainable Forestry Initiative and may not be supported by your particular printer.
Frankly, I am not sure what else to suggest. 😞 Do you know anybody else who has printed on one of these forms, and if so can you ask them how they did it?
