I think many of you might met the same issue as me - HP Scan "DOES NOT WORK AT ALL", not know since when or what updates to made this happen, however, the answer I can found from HP are all sucks - Not helps at all.
But after seeking and did some testing, I figure out the solution, it might helps you to solve your problem - You use HP Scan to scanned a doc but there's no image ourput.
Here it is:
1. Stop use HP Scan anymore, because it's default scanner setting is not works at all!
2. Use "Windows Fax and Scan" from the progrm menu list and launch it.
3. Click the "Add a new scan"
4. On the "Select Scanner" dialog box, DON'T select HP LaserJet MFP M28-M31 (NET), SELECT the other one HP LaserJet MFP M28w (5F69FB) ** your displayed name might be different, then click OK to next add new scan dialog box
5. Make sure the scan settings and click "Preview" or "Scan", then you can get you doc/photo be scanned.
That's all! And this helps me to resolve the scanning issues for months. Hope above steps helps you too.

Here comes another solution by using HP Scan but changing the WIA name.
1. Still use "Windows FAX and Scan/Add new scan" to get the Scanner name. Here at my computer, it displayed as "HP LaserJet MFP M28w (5F69FB)"
** The name listed on HP Scan is "HP LaserJet MFP M28-M31 (NET)", and which is not work one.
2. Go to below location, and find the "ScanApp.ini" file and open it.
C:\Users\Venson\AppData\Local\HP\HP LaserJet MFP M28-M31\
3. On the top section of "[Device]":
a. add a line of "WiaModel3=HP LaserJet MFP M28w (5F69FB)",
b. change the "NumWiaModels=" number to 3
c. Add ";" to remark WiaModel2=HP LaserJet MFP M28-M31 (NET) line. (Disable it).
Please refer below as as reference.
[Device]
Model=HP LaserJet MFP M28-M31
TwainModel=HP LJ M28M31
WiaModel1=HP LaserJet MFP M28-M31 (USB)
;WiaModel2=HP LaserJet MFP M28-M31 (NET)
WiaModel3=HP LaserJet MFP M28w (5F69FB)
NumWiaModels=3
4. Save it and turn on the HP Scan application. Now you will see the dialogbox header has been changed to "HP LaserJet MFP M28w (5F69FB)" (In my PC as example)
5. Start a new scan, and you will get the scanned image displayed.
I have to say I didn't see any HP people post such solution to make it works and save user's time and effort, but this is what i tried and works, hope above steps can helps you resolve the scanning issue and get what you want scanned.
