I have set my printer to Netherlands / Dutch, every thing in the settings is A4.
and still. the scan shortcuts (like to OneDrive) refuse to have de default on A4 and 2-sided.
There is no setting anywhere in the app or advanced setting i could find where to change this, yes 1 possible option on the printer "scan to cloud" -> "edit" where i can change it, but it does not save it once i changed it it just reverts back to 1-side / letter.
every time i need to change this manually, very very annoying.

OK - I'm guessing you have a dual purpose HP Smart Shortcut created for Scan to Cloud and Save plus a Print option in the same Shortcut.
Ideas...
If not done, (re)check your EWS Default Print Options to be sure what you are expecting is the same as the what the printer expects.

Open your Browser > Type / enter the Printer IP address into URL / Address bar
Section Paper > Trays > click on Tray to configure
Next (in addition and as available)
Section Print > Default Print Options > Select default tray

If prompted for a password, enter the default printer PIN available on the label inside the printer access
door. If you have changed the password, enter your new password

At the moment, I don't have a dual-purpose Shortcut example for Save to OneDrive and Print.
I do have two separate examples of what the Shortcut(s) Save and Print options look like.
Yes, if your one shortcut is set to Save and (then) Print both of the actions should work.
I do not see Paper Size / Type Setting in the Shortcut.
Content type impacts how it can be printed (as do Paper Size and type).

EWS Default Print Options : all Netherlands, all Dutch, all on A4 (goes for all setting in the EWS where i can set language and paper size)
Tried all proposed in your post, even deleted everything, uninstalled all applications, factory reset, tried all setting again, made the shortcut again, still when everything is in NL / Dutch / A4, the shortcut "scanner -> cloud -> OneDrive" keeps defaulting on 1-side / Letter 😞

Well, I don't know...
I'm perhaps missing something or my notes are incomplete.
The dual purpose Shortcut might be tripping over it's own settings.
One part of the Shortcut must be set to extract the Scan content for the Save action.
The next part of the dual purpose shortcut (presumably) prints the content using the Print settings.
Scan includes its own Default Job Options
EWS > Section > Scan to Network Folder > Default Job Options
Perhaps (Re)Check the "Original Sides" and "Original Size" settings.

Perhaps separate the Shortcut into two separate shortcut actions:
Create Shortcut to Scan Content and Save
Create Shortcut to Print
Scan - run the Scan Shortcut -
Before leaving the job entirely, loop back around and run the separate shortcut to print.
What does that prove - maybe nothing though it might help point to where an action is failing.
I will post back if I run across a better example in my notes.
