I bought a new Polycom VVX 250 IP phone from Amazon.  The default User password 123 works, but the the 456 Admin password does not work.  I attempted a factory reset by power off and on while hold the back and home keys, but that did not workeither.

Welcome to the HP community.

 

Unfortunately, your post does not contain the currently used Software version for the HP Poly product you use. 

 

This is important, especially in the future, when someone finds this post in days, months, or years.

 

Please do not simply edit the original post but reply with the versions.

 

In the FAQ or the Read 1st section, we remind users why this is important.

Sep 19, 2022 Question: Why does a Phone return/revert to a previous unknown password even after a factory reset?

Resolution: If a phone is provisioned either by a local server/ZTP/LENS and/or a DHCP option the phone will simply load the old configuration.

On phones with access to the low-level GUI (option 4 >here<) once the password is reset either ZTP can be disabled or the Boot Server can be set to static. For phones that do not have such an option (CCX, Trio, D230, Rove or Edge E) please check with your IT team or whoever set up the phone.

 

Both can be found utilizing the community search.
