Desktop displaying Select Keyboard & CP loads from x:

My desktop on power up keeps displaying "choose keyboard layout" then once selected only displays "Troubleshoot" > "Recovery Manager" & "Advanced Options." Attempted System Restore, StartUp Repair. and System Image Recovery, all displaying "unable to complete." Rwset PC is not even coming up as an option, so I did the whole unplug & restart; no success. Did hardware diagnostic and failure code displayed failed hard drive, but I am feeling like it really isn't being no changes were made to pc. I noticed in Advanced Options, Command prompt is loading from X: drive, so I feel as if for some reason it cannot find/load c:

Sorry, I don't understand this well.
Did you take the test and get a failed disk message?

Yes. My apologies. Computer displayed "hard drive failure" and noted the hard drive may need to be replaced. Provided error code.
