The print engine and formatter have lost communication with each other.
- Turn the power off then on by pressing the power button and holding it in for more than 3 seconds.
- Turn the power back on using power button.
- If the issue persists, have the customer remove the formatter board.
- Inspect the contacts for contamination or damage.
- Clean the contacts with a damp, lint free cloth. Ensure the contacts are completely dry before reins...
- If the contacts are found to be damaged, replace the formatter board.
Since you have replaced the formatter and you still get this error, it means that the Interconnection Board maybe faulty. It's the board where the formatter connects to.
As long for the usb port disabled "error", try enable it through EWS (Embedded Web Server) or through the printer menus (probably under the Administration->General Settings).
These are my personal suggestions, I hope you good luck.

Connect the printer to the network. Let it acquire an IP address, then use that address in a browser to access the EWS.
In there somewhere, usually under the Security tab you can find an option about USB Device enable/disable.

From the event log you have posted, I can see the 61.00.02 error in more details (C6FD0503). This error relates to "Printbar malfunction" and the action suggested by the HP error code manual is to "Replace engine with Mech Kit".
Recommended action
1. Turn the power off then on by pressing the power button and holding it in for more than 3 seconds.
2. Turn the power back on using the power button.
3. Repeat steps 1 & 2 again if the error remains, then:
1. Check the installed firmware version and update the product firmware as needed.
Even with the printer on error condition you can still navigate the menus and FW version
2. Turn the power off and remove the formatter, check the formatter thoroughly for any loose connections and reseat them, then reinstall the formatter.
4. If the error persists, replace the Mech Kit.
5. If the error persists, turn the power off and remove the formatter, check the formatter thoroughly for any loose connections and reseat them, then reinstall the formatter.
6. If the error persists, replace the Mech Kit.
7. IMPORTANT: Print the event log OR access the event log through the EWS. Under the Description or Personality field, look for the 8 character fail code values corresponding to any 61.00.0X errors and log them into the case notes. These values are critical in helping the division understand the failure and in debugging potential field issues.
I am not very familiar with officejet (inkjet) models, so maybe someone else can tell you what to do next or what the Mech Kit involves.
BUMP from me.
Cheers!

Before you go any further, check the status light on the formatter. It should be green and flashing, if not, then formatter is failed.
Two notes, based on a quick look at some tech documents. If the printer has an additonal tray on it remove it. Also ensure that you have no usb or network cable attached as a stale print job may be triggering the 49 errors.
