From the description and the amount, I am guessing this might be a Smart Friends service account.

These do not grow out of trees (pop up unexpectedly) - you have to purchase the service.

 

That said, you should / could examine your purchase history.  For example, search for emails that you have archived for finances / charges.

Contact Care Pack / Smart Friends Support.

 

Care Pack - Optional Coverage Plans

Select Region (top right) > Enter your product number in the form to view available Warranty plans for your device
