I tried something new today in BIOS (1.03.02) today.

Turn off boost converter

Force Customized Fan Control Options to 1

Turn off AMD Core performance boost

Turn off HP motion sensing cooling mode

 

Leave ZBook alone and windows in desktop doing nothing. Observed that fans eventually turn off after awhile. However, fans will now constantly spin up for a few seconds and then turn off again even if the laptop is doing nothing in desktop. Again, this made me think either BIOS or some sort of driver is mis-behaving.

The behavoour you describe may be correct operation.  When idle this lap top only uses a few watts and will only very slowly heat up, once the temperature gets high enough the fans will come on and cool the chips down again then switch off and repeat.  This is called hunting and results from hysteresis.  Most central heating systems do it.  It is also quite possible that even when idle every so often the lap top will go and check [poll] for something - e.g. have you received any email, upping the activity level.

Another issue I noticed with this lap top is that all the vents are on the bottom (my old HP DM1 vents at the side).  If laid on a soft surface, like a chair, the vents will be obstructed redcuing cooling effieciency resulting in a temperature rise and fast fans.

I am not sure, mate. My company issued Thinkpad P1 with i9-13900H + RTX-2000 has no such issues. Fans are nearly silence when running in Best efficiency mode. For my own Ultra G1a, its fans goes even if I set to best efficiency mode. So is like the fans full speed for a few seconds off and repeat. Super annoying. You may argue that the P1 had larger space to work with the heat, but the behaviour does not make sense. P1 behaviour is way more sensible. Constant low and quiet fans speed.
