Have you tried plugging a usb flash drive into all of the ports?  What is the result?

Is there a failure at the USB 3.0 port or is it the USB 2.0 ports?

It would be more than a bi strange if USB 2.0 and 3.0 ports have failed.

The failure code is a USB Failure (unspecified).

If the ports do not work, the only resolution is to replace the systemboard.
