Product: OfficeJet Pro 8028 All0in-One

I purchase the 8028 from Costco last month, and it included  4 Months HP Instant Ink Delivery Service. I have misplaced the prepaid enrollment card that came with the printer. Can you please tell me how I can redeem the Instant Ink credit? My Serial Number is [personal info removed]. Thanks.
