Hi,
By 8 x 4 inches, do you refer to a 4 inches width or lengh?
Be aware that a 4 inches lengh is not supported by the hardware (5.0 to 14.0 inches) and therefore will not appear, if this is the issue indeed simply change between the height and width settings for the specific form and change the paper orientation..

Hi,
Follow these steps to create a custom paper size for the printer for the printer:
http://support.hp.com/us-en/document/c03890386/
Only supported paper sizes will appear within the printer driver, the printer support 76 to 216 mm (3.0 to 8.5 inches) wide and 127 to 356 mm (5.0 to 14.0 inches) long

I have already created an 8x4" via
Control Panel / Devices & Printers / Print Server Properties / Forms
I can see the custom size using cutePDF, but none of my custom sizes are available on my HP8610
or am i missing something??
