Bad print quality

Printed out a Form 1099R for my tax return.  The printout added sidebars which do not appear on the online PDF copy and parts of the printout were clear and the rest were either fuzzy or looked like double print.

Bad print quality.

No error message.

Plenty ink in cartridges.

Printer Status Report shows everything perfect.

I have gone through your Post and would like to help

Are you printing the Form from an online website or from an App?

If yes, try downloading the form and then print it directly from your MAC

Also ensure your Mac and Printer have the latest updates

I tried printing it from the website.  

I have just spent several frustrating minutes trying to download the form using the directions on the Safari help button.

Also I tried updating the printer, but no updates are available for a 19 year old printer.

My MacBook Air is updated.

Thank you for your suggestions. However, I just bought a new printer, which I'm hoping will solve my problems.
