Prints partial lines 4 times.

My printer keeps printing partial lines 4 times.  And then another partial lines 4 times, etc, etc.

I have gone through all the steps that HP is giving.  I have downloaded drivers.  I have done at least 10 Printhead Alignments (all failed).  

Was working fine until a couple days ago.  Said the printhead could not be located, or something along those lines.

Here is what it printed out when I did the printhead cleaning.

What is happening with my printer?
