To me it looks like under the title, number 1. and its text is missing.
It also looks as if some text is missing under the ColorLok circle.
The complete text for Number 5 is not there.
Above the black line & below the green box, it looks like some text is missing too...

I have run the print head cleaner numerous times, along with aligning the heads too... I don't know how else to fix this issue??

Looks like one segment of a black ink cartridge isn't printing, replace the black ink cartridge if the cleaning isn't helpful.
