Headers and Footers are controlled in the Print settings in the browser.

Examples:

Chrome > Settings > Print > More Settings > Select headers and footers Toggle On / Off

Print_Preview_Chrome_More_Settings_1Print_Preview_Chrome_More_Settings_1

Edge browser appears to work about the same as Chrome, except you do not have to click "More Settings".

Open Web page > Click Settings (three dots in a row - upper right Edge window)

Print > Under Headers and Footers, Select "On"
