Until today I was able to scan via JPEG mode and insert that scan into Word document
today JPEG produces only two small fragments of the total image
am able to scan via PDF mode but don't know how to insert that into Word
(using HP 4500 Envy print/copy/scanner)
also, hitting OK once produces two or three separate scans on computer

Hi @Dan45H_,
Welcome to HP Support Community.
Thank you for posting your query, I will be glad to help you.
The HP ENVY 4500 E-All-In-One Printer should be able to scan documents and images in both PDF and JPEG formats. If you are experiencing issues scanning in JPEG format, here are a few things you can try.
- Check the settings: Make sure that the scanner settings are configured to scan in JPEG format. This can usually be done through the scanner software or the printer's control panel.
- Update the driver: Ensure that you have the latest driver software for the printer installed on your computer. You can download the latest driver from the HP website.
- Check the file size: If the file size of the scanned image is too large, it may not save in JPEG format. Try reducing the resolution or compressing the image to a smaller size.
- Try scanning from a different device: If the issue persists, try scanning the image from a different device to see if the problem is with the printer or the computer.
I hope this helps.
Take care and have a good day.
