I have brought your issue to the attention of an appropriate team within HP. They will likely request information from you in order to look up your case details or product serial number. Please look for a private message from an identified HP contact. Additionally, keep in mind not to publicly post personal information (serial numbers and case details).

So it should be all sorted out. I had to submit the form (https://store.hp.com/us/en/cv/risk-free-care-pack), mine was outside the time frame so it was rejected but I asked for it to be escalated and they gave me an exception after I explained the situation.
Not sure if you are outside the time or not. The form didn't list my laptop, so I originally thought it was the wrong one, but it seemed fine. Make sure to include the documentation that they ask for in the form as well.

thank you, for your reply, docjosh. i havent been online much because my husband is having a difficult recovery after bypass surgery, so i didnt see your response.
it sounds like you finally succeeded in getting the rebate you deserved. congratulations!
i have a couple of weeks left in my 90 day post-expiration period. today i filled out the same form as you, but couldnt submit it because it asked me to create a free pdfiller account and give them my credit card before i could email the form to HP. i wasnt willing to do that so sent in an email asking if there is any other way to submit my request. so far no response.
it shouldnt be this difficult.
