Hi,
You may try erasing the hard-disk and re-flashing the latest firmware and check if that may help, else the hard-disk might failed, as a reference see below support document:

I would run the harddisk diagnostic and see if it passes. We have seen a fair number of current generation devices including M528s fail with a bad harddisk. The common factor in all of them was the Toshiba drive that was installed. Diagnostics are accessed through the preboot menu under the administration menu.
