I am having the problem, different size paper 13.9mm x 21.59mm. I have created the new form in Windows 10 using the "server" update function. After creation, the new form exists there following a restart. I then proceed to Excel - 97 and can not find the form just created on the server. I have tried the "Advanced" options as suggested but can not find the new form there. I have repeated the process attempting to print from Gmail and trying to select the new form but, it is not there either. FYI printer is an HP OfficeJet 7500 E910. Any help is appreciated.

Thanks for the response.
In this case, I recommend you delete the driver and root level and retry, follow the steps below:
In Windows, search for and open Programs and features
2.) Select your HP Printer
3.) Select Uninstall
4.) In Windows, search for and open Devices and printers
5.) In the Devices and Printers windows look for your HP printer. If you see it listed right-click on it and choose `Delete' or `Remove Device'
6.) Open up the run command with the "Windows key + R" key combo.
7.) Type printui.exe /s and click Ok. (Note: there is a space in between "printui.exe" and "/s")
8.) Click on the "Drivers" tab
9.) Look for HP Printer driver. If you see it Click on it and click Remove at the bottom
10.) Select Ok
11.) Select Apply and Ok on the Print Server Properties windows
12.) Close Devices and Printers
Once done, follow this link do a quick search with the product number of the printer, select the right operating system, and install the full featured driver software. 🙂

I did attempt this solution. Windows now recognizes the new form but, applications don't pickup the change. When I select forms type inside any application the new form does not appear. However, I did find a setting on the printer which will cause it to ignore form width. With this turned off I am able to print on the smaller paper eventhought the application paper size does not align with what is actually in the printer. This work around is working so I am going to close the ticket. Thanks for your help sorry we could not find the root of the problem but, I can now continue with my project.
