I'm having similar issue with my officejet 4500. at warm up the printer automatically feeds a sheet of paper and it proceeds to come out (half way thru) and just gets stuck. it's doing this at startup even though I've cleared all the jobs from the queue. before it was just doing it when i attempted to print. it stays stuck with "printing" on the display screen until it goes into powersave mode. I've managed to go through a few "pager feed cleaning"s using the HP Solution Center, but nothing occurs for "back of page smear cleaning". the tricks you list above do not work as my printer just gets stuck with "shutting down" on display screen when i push the power button simultaneously with cancel button or color copy -- before going back into powersave mode instead of shutting down.

I suggest you perform a printer reset
(1) Turn the printer on, if it is not already on.
(2) Wait until the printer is idle and silent before you continue.
(3) With the printer turned on, disconnect the power cord from the rear of the printer.
(4) Unplug the power cord from the wall outlet.
(5) Wait at least 60 seconds.
(6) Plug the power cord back into the wall outlet.
NOTE: HP recommends connecting the printer power cord directly to the wall outlet.
(7) Reconnect the power cord to the rear of the printer.
(8) Turn on the printer, if it does not automatically turn on.
The printer might go through a warm-up period. The printer lights might flash, and the carriage might move.
Wait until the warm-up period finishes and your printer is idle and silent before you proceed
