Mac OS X 10.5 Leopard and older

I have a problem with printing (stripes on print). Contacted HP via the support form, and received a mail that I will receive a new cartridge. So far so good.

 

In the e-mail HP requests additional info (adress details), replying to the mail does not work, and there is also no other way to share the info with HP. How to proceed?
