Ther are four separate USB ports on rear of Printer .  group of two ports on top and another group of two below.

Which of 4 ports should I use to connect to printer.  I intend to use fax capability of printer.  Best, Aaron

Hi Aaron,

There is only a signle USB [pet at the back of the printer, not 4.

 

To connect the printer to you PC, plug the USB to the Rear USB Port, as shown below:

To connect the device as a Fax, plug a Phone Cord from 1-LINE to a wall phone socket.
