Looks like it does not support printing on index cards. Its specs (Paper handling section):

This is very strange for this type of printer.

Hmm... that is weird (unexpected).
Datasheet says it prints "cards".
Extracted data files from the Web Pack includes 4x6 Index Card and 5x8 Index Card and other bits that appear to indicate the printer can print on the "card" Media type.
Specification sheet is one of the "new style" documents -- I find these harder to read. Grumble.
Perhaps the information is incomplete??
IF that is the case..and the printer does indeed print on Media type card, then:
The issue remaining is whether you can print on card size 3x5 inches.
I cannot definitively answer that question. The 3x5 inch size might be just "under" the minimum indicated in the Datasheet -- or it might be OK.

If the Custom Form setup is available, find it:
Control Panel > icon view > Devices and Printers >
Left-Click on your Printer one time to highlight >
Left-Click Print Server properties
Hints
Select a standard form template that is of the same Media type as your new custom form.
For example, if the new form is for a "card", start with a "card" template.
Change only the paper "size" parameters for the new form -- leave the margins "as is" (at least until you know whether this is going to work at all -- after that, try what you like...)

I clicked on the suggested link, and under the heading "Paper handling - ADF" it shows: "Media sizes, custom 76 x 127 to 216 x 356 mm (3 x 5 to 8.5 x 14 in)." This would indicate it will handle a 3 X 5 card, but I can't firgue out where to load it. The paper width-guide in the paper tray won't collapse down to 3". If you try putting in the index card and collapsing the paper width-guide to 5" you receive a msg stating the paper is too short and the minimum length is 4.9" and the card length is only 3". The photo tray also will not accomodate the 3 X 5 index card. So the question is, where and how do you load the index card.

It would appear that the information conflicts with the physical limitations of the paper guides.
(I cannot test / confirm this for you, I don't have this printer on hand.)
If the paper guides do not adjust, then it is not likely you can test / print the settings.
It is likely that @banhien has, in fact, provided the correct answer.
If the printer is under warranty, contact HP and pose the question.
Having barged in unannounced in the first place, I will now take my leave.
