I tried to scan a document, but all I got is this......Could anyone provide me some tips to fix it?

It's a scanner defect then.  Hardware needs to be serviced.

 

Contact HP for support if the printer is still under warranty:

From the picture, it's likely that the scanner is defective.

 

Is it seen on both scan and copy?
