unable to add pages to scan, + button gone

Not sure when it disappeared, but I used to have the + button when I wanted to scan multiple pages into a document. Now it's gone. I do not see an option that allows me to scan more than one page into a file....any suggestions? I'm wondering if this is related to the recent Windows update?? I ran the diagnostic on my printer/scanner, all is well there. And I do not see that I am missing any updates on that side.

Are you set to 300 dpi or less?   Scanning from glass or feeder (if you have one)?

I did try it at 200dpi, but would actually like to get it at 600dpi (but anything would be great at this point!), and yes, I am using it on the glass scan bed.

Driver updates: HP Customer Support - Software and Driver Downloads 
Once it's downloaded and installed, go to Start > All Programs > HP > HP Scan Extended > Run "HP Scan Extended"
