How do I configure CUCM 14 and the CCX 500-M phone (v7.0.3.0228) so it will register with CUCM?

From our general VoIP FAQ:
Oct 7, 2011 Question: Can I register, or is my Poly Phone compatible with a “XYZ” SIP Server?
