I scan document, it cuts off page at bottom and lowers the top of document automaticly and rotates once I attach file to send. I set paper size to 8.5x11, black and white, I've unpluged to reset but something in settings is not correct. How do I correct this issue, Need file saved in PDF format.

Thank you for posting your query. I'll be more than glad to help you.
I understand that you are facing scan quality issues with your HP OfficeJet 5745 e-All-in-One Printer wherein the document is not scanned properly.
This issue could occur if the scan settings are not correct or the drivers may not be installed properly.
Let's go through a few steps that may help to fix this issue.
And here are a few steps in the link: Fixing Scan Quality Issues
Note: Please follow the step 4 under HP Printer Assistant.
If the issue still persists, then I suggest that you uninstall and reinstall the drivers by following the steps below:
Step 1: Uninstall the Drivers:
Please follow the steps provided to uninstall the printer software in the link: Uninstalling the Printer Software (Windows)
Also, please follow the steps provided below to completely remove the printer software from printer properties.
Steps to remove printer driver files from Print Server Properties:
1. Open the Print Server Properties dialog window by doing any one of the following:
(a). Press WINDOWS-R on the keyboard to bring up the Run window, type in 'printui /s /t2' (without the quotes), and then press Enter or click OK.
(b) From the Start menu or the Start screen and open Devices and Printers. Select any printer and then click "Print server properties" at the top of the window. Click on the Drivers tab.
2. Select the printer driver you wish to uninstall.
3. Click the Remove button.
3. Select "Remove driver and driver package" and click OK.
Step 2: Download and install the Drivers from the link: Full Feature Software and Drivers

I clean the rollers and glass and unpluged and that did NOT resolve the issue, but...
Not sure which of these to remove, please advise, this is what i see when going to printer properties, Please advise what to remove and instructions on what and how to re-install

I tried to take a screen shot but could not paste after using Snipping Tool so you could see what i see, thanks for all your follow up help

I suggest that you keep the first one HP Officejet 5740 series and delete the rest.
